CANCELLATION or REFUND POLICY

Our patients are the most important part of our practice. This cancellation policy is put in place for our patients. Our appointments are reserved just for you. Related to this we want you to understand that it is also a potential appointment for someone else who may have wanted that particular time/date.

 

 

Surgical Procedures/Surgery

 

There is a non-refundable scheduling fee and deposit of $2000 for any surgical procedure. If you choose to cancel your procedure for any reason the $2000 fee will remain non-refundable.

 

• Cancellations made less than ten (10) business days from your date of surgery will result in 50% charge of the total surgery/procedure fee. (Including, laser procedures, injectables, etc.).   This only applies to the surgeon's fee.  The surgery center and anesthesiologist would have a separate policy.

• Cancellations one day before or on the day of the procedure will forfeit entire balance paid.

• These fees may be subjected to change.

 

The physician’s fee does not include hospital, anesthesia, pathology, laser, post-operative materials/garments or implants of any type.
